A Care Coordinator is a registered nurse and/or licensed practical nurse who aids patients and families. this care team works closely with patients and their providers to help make and reach health goals.

• Build relationships with patients and their supportive caregivers
• Provide patients and families with education and supportive care options
• Help patients and families understand specific health concerns and treatment plans
• Assist in understanding medications and their importance
• Assist with scheduling appointments and transportation
